Throughout its duration, Spring worked directly with and took referrals from professionals (incl. GPs), primary care networks (PCNs), and local services who identified individuals that could benefit from Spring’s service.
What is Social Prescribing?
Social prescribing is an approach to helping improve people’s health and wellbeing.
Its core principals are around:

Building a strong relationship with an individual:

Listening to what is important to them; and

Linking them with appropriate community groups, services & activities.
